Interprofessional Collaboration for Improving Patient
and Population Health Outcomes
Demonstrates work as a member and leader of interprofessional teams; communicates, collaborates and consults with other health professionals to manage and coordinate care across systems and environments.
Interprofessional collaboration is essential for improving patient and population health outcomes. This competency was met through my Quality Improvement (QI) Project in Continuous Quality Improvement and Outcomes Management (NURS 5056) and even more so through my Transformational Strategies Practicum (NURS 5058. 5059, 5060) with my Strategic Project: Improving Access to Oncology Care.
With these two projects, I demonstrated work as a member and leader of interprofessional teams. My QI project addressed improving patient wait times by implementing standing order sets upon triage in the emergency department. I collaborated with administration, nursing, physicians, and support staff. My strategic project fostered development in communication, collaboration, and consultation with other health professionals in order to manage and coordinate oncology care.
This competency was also developed throughout my graduate education at King University with group projects such as our health information technologies project, business plan, and more. As I have mentioned in a former narrative, I am a planner by nature, and this is a strength when working with interprofessional teams. I find it easy to delegate and assign tasks as well as complete tasks assigned to me. At times, I get caught up in the details and overwhelmed by the big picture, that is my weakness. I truly belive and try to live by the motto "a day at a time" and I try to prioritize according to deadlines.
